Job Description: We are seeking a skilled and compassionate Nurse Practitioner to join our medical practice in PA. Our focus is on providing high-quality care to patients in skilled nursing facilities and assisted-living facilities.
Position Details:
- Part-time role: 2-4 shifts per month
- Paid per diem
- Opportunity to make a meaningful impact on patient care
Responsibilities:
- Conduct comprehensive patient assessments and physical examinations.
Develop and implement treatment plans in collaboration with supervising physicians. Perform diagnostic procedures and interpret laboratory results.
- Provide patient education on health management and disease prevention.
- Administer medications and treatments as prescribed.
- Document patient information accurately in electronic medical records (EMR).
- Assist in the management of patients in nursing homes or long-term care facilities.
- Stay current with advancements in medical practices and technologies.
Ideal Candidate:
- Holds current certification and licensure in PA.
- Experience in post-acute care or skilled nursing facilities (preferred but not required)
- Strong clinical, communication, and interpersonal skills
- Experience with telehealth platforms and remote patient monitoring.
- Ability to work effectively within a team environment while demonstrating excellent communication skills.
