Licensed Practical Nurse - Home Care

Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic home care team. In this vital role, you will provide high-quality nursing care directly in patients’ homes, promoting health, safety, and independence. Responsibilities

• Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
• Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards.
• Manage complex wound care, including dressing changes and infection control measures, especially for patients with chronic wounds or post-surgical needs.
• Assist with activities of daily living (ADLs), such as mobility support, catheterization, tube feeding, and personal hygiene to enhance patient comfort and safety.
• Coordinate with physicians, case managers, and other healthcare professionals to develop and implement individualized care plans aligned with patient goals.
• Educate patients and their families on health management strategies, medication adherence, and disease processes like Alzheimer’s disease or dementia care.
• Maintain accurate medical documentation in compliance with HIPAA regulations and ensure timely reporting of patient progress or concerns.

Skills

• Proven experience with Epic EMR systems or similar EHR platforms such as eClinicalWorks or Athenahealth for documentation and patient management.
• Critical care experience including ICU or emergency medicine settings is highly desirable for managing acute situations effectively.
• Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques.
• Skilled in IV insertion, catheterization, phlebotomy, medication administration, and sterile technique procedures.
• Experience working with individuals with developmental disabilities or those requiring specialized memory care such as Alzheimer’s or dementia care.
• Ability to perform patient assessments accurately while demonstrating excellent communication skills for triage and health coaching.
• Familiarity with home & community care standards, post-acute care protocols, hospice care practices, and infection control procedures.
