RUSSIAN SPEAKING SOCIAL WORKER AT ADULT DAY HEALTH CARE CENTER.
Looking to hire Social Worker with a Master's degree in related field (details below) or IF YOU HAVE ONLY Bachelor's degree, it has to be in Social Work ONLY . Five days a week Monday to Friday. New graduates are welcome.
The hours for the position are Monday thru Friday 8:00 am-2:00 pm
Job Summary:
- Interview and screen all referrals to determine the general appropriateness of the prospective participant for the full assessment process and adult day health care participation.
- Provide signed and dated documentation for all services performed the day the service
- Provide for periodic reevaluation and revision of plan of care.
- Provide counseling and referral to available community resources.
- Promote peer-group interaction and relationships through discussion groups.
- Serve as liaison with the participant’s family and home.
- Serve as liaison with other community agencies that may be providing services and coordinate all services to a participant and work with these agencies to coordinate all services delivered to the participant to meet the participant’s needs and avoid duplication.
- Provide discharge planning for participants.
- Supervise and train assistant social workers.
- Other duties as assigned.
Qualifications:
- Master’s degree in social work from an accredited school of social work; or
- MA in psychology, gerontology, or counseling from an accredited school and one year of experience in aging, health, or long-term services; or
- BA in social work from an accredited school with two years of experience in aging, health, or long-term care services, or Licensed by the California Board of Behavioral Sciences
Salary : $28 - $30
