- One year of experience providing services to individuals with disabilities, preferably working with individuals with traumatic brain injury.
- Knowledge of community resources and means of accessing these resources.

- Maintains confidentiality of information relating to individuals and their families in accordance with agency policy.
- Participates in conferences, workshops, department meetings, in-service and on-line training as needed or recommended to enhance job skills and personal growth and development.
- Adheres to agencys policies and procedures as well as department guidelines.
- Accesses and maintains passwords and log ins for all required electronic health records, agency email, virtual trainings, and electronic employee records as required by the program.
- Functions as the primary assistant to the Program Specialists.
- Reviews each individual plan, safeguards/PPO and any other relevant information related to the care of individuals at least annually and as changes occur.
- Implements each program participants therapeutic and behavioral plans.
- Works with individuals on special instructional projects to achieve goals on each individuals program plan.
- Provides the Program Specialist/ Supervisor with information about program participants, which will assist in the development of appropriate program experience for all participants.
- Completes required documentation about individuals performance according to established guidelines and timeframes.
- Assists in the development of instructional materials.
- Assists the Program Specialist in related instructional work as required.
- Assists in maintaining the cleanliness and the orderly appearance of program equipment, supplies and program areas.
- Attends all meetings as required and appropriate.
- Leads planned small group activities including but not limited to in house activities and community activities.
- Drives the agency vehicles as required to facilitate community-based activities.
- Operates wheelchair lifts and ramps safely and in accordance with operating procedures in agency vehicles.
- Maintains a safe environment at Happiness House and in the community.
- Provide the participants with assistance on ADLs including but not limit4ed to, recognizing safety hazards, transferring, toileting, changing, ambulating and feeding as required by individual need.
- Functions as a positive, contributing member of the TBI/NHTD/OPWDD program team.
- Performs other duties at the discretion of the Program Supervisor and Director or his/her designee.
