Job Summary:
The Pharmacist provides professional pharmaceutical services to patients of the health center, ensuring safe, effective, and affordable medication use. This role supports the mission of HAPPI Health by delivering culturally competent care and contributing to patient education and chronic disease management.
Essential Duties and Responsibilities:
- Dispense prescription medications accurately and in compliance with applicable laws and regulations (state and federal).
- Review prescriptions to ensure proper dosage, interactions, and therapeutic appropriateness.
- Counsel patients on medication use, potential side effects, interactions, and storage.
- Collaborate with medical, behavioral health, and nursing staff to support integrated care models.
- Provide medication therapy management (MTM) services to improve patient outcomes.
- Participate in 340B drug pricing program compliance and inventory management.
- Monitor inventory and ensure appropriate storage of pharmaceuticals.
- Maintain accurate records in the electronic health record (EHR) system, Athena.
- Support quality improvement initiatives and clinical outcome goals.
- Stay current with continuing education and federal/state pharmacy regulations.
Qualifications:
Education and Experience:
- Doctor of Pharmacy (PharmD) degree or equivalent required.
- Minimum of 1–2 years of experience in an outpatient, clinical, or community health setting preferred.
- Experience in FQHC or 340B program environments strongly preferred.
Licenses/Certifications:
- Current and unrestricted Pharmacist license in Alabama
