The Family Nurse Practitioner (FNP) provides comprehensive, high-quality primary care to patients across the lifespan, from newborn through geriatric, in a Federally Qualified Health Center (FQHC) environment. Working under a collaborative practice agreement with HAPPI Health’s Chief Medical Officer and supervising physician team, the FNP delivers preventive, acute, and chronic care management to a medically underserved patient population. This role is central to HAPPI Health’s mission of expanding access to affordable, whole-person care, and directly contributes to the organization’s clinical quality, productivity, and community health outcomes
Key Responsibilities
• Provide direct primary care services — including health assessments, diagnosis, treatment, and follow-up — to patients of all ages, from newborn through geriatric populations.
• Maintain a full clinical schedule, averaging approximately 25 patient visits per day, consistent with FQHC productivity and UDS (Uniform Data System) reporting expectations.
• Order, perform, and interpret diagnostic and laboratory studies; develop and implement individualized, evidence-based treatment plans.
• Prescribe medications and therapies within scope of practice and in accordance with the collaborative practice agreement on file with the Alabama Board of Nursing / ALBME Joint Committee.
• Complete and close patient charts the same day or no later than the next business day, in accordance with documentation and coding compliance standards.
• Utilize the organization’s electronic health record (Athena preferred) to document encounters, manage e-prescribing, track quality measures, and support care coordination.
• Collaborate with the Chief Medical Officer / supervising physician on complex cases, in accordance with applicable scope-of-practice and supervisory requirements.
• Identify and manage complex or co-occurring medical and behavioral health conditions; refer to specialty, behavioral health, or social services as clinically indicated.
• Participate in integrated care initiatives, including behavioral health integration, care coordination, and chronic disease management programs.
• Support population health and quality improvement activities, including UDS measures, HEDIS metrics, MIPS, and Patient-Centered Medical Home (PCMH) standards.
• Provide patient and family education on preventive health, chronic disease self-management, and wellness.
• Maintain compliance with HRSA requirements, Alabama Board of Nursing regulations, HIPAA, infection control, and all HAPPI Health policies and procedures.
• Participate in peer chart review, clinical staff meetings, and continuing education as required for FQHC and licensure compliance.
