General Description: This is a professional position that is responsible for providing case management services for children and adults receiving behavioral health services (to include mental health and/or substance use disorder). The incumbent performs complex tasks to determine, plan and manage services.


Organization: The Case Manager position is part of Hanover County's Career Development Program (CD). The Case Manager Ladder has three (3) levels: Case Manager I, II, and III. The Case Manager reports to a Case Manager Supervisor, Clinical Supervisor or Program Coordinator and supervises no staff.


Essential Functions:

  • Obtains and assesses individual and family social, economic, occupational and physical and mental health information to develop a person-centered individualized services plan (ISP) based on strengths and needs.
  • Meets with individuals and families in their homes, at school, in the community, or in the office.
  • Monitors individual’s changing behavioral health needs, including health status, and coordinates services accordingly.
  • Links and assists individuals in accessing medical, housing, transportation, social, educational and other supports and resources to address their needs.
  • Develops and implements crisis plans to help individuals remain in the community when appropriate.
  • Monitors service delivery through contacts with service providers and performs site visits and home visits to ensure that services are being provided in accordance with the ISP.
  • Maintains accurate records in accordance with local and State guidelines.
  • Performs related work as assigned.

Knowledge, Skills and Abilities: Requires knowledge of community services and resources; serious mental illness, developmental disabilities and substance use disorders; assessments person-centered service plan development and evidence-based treatment approaches; as well as knowledge of Medicaid and Department of Behavioral Health and Developmental Services policies and regulatory guidelines. Must be able to assess and document individual service needs for services and supports, coordinate services across providers, develop and maintain strong relationships, and communicate effectively, both orally and in writing. Must be able to work and make judgments independently, as well as function as part of a team.


Education, Experience and Training: Bachelor's degree required in a related field (master's degree preferred) with at least one (1) year of related work experience – OR – Any equivalent combination of education, experience and/or training sufficient to demonstrate the knowledge, skills and abilities is acceptable. Must meet the eligibility requirements for and/or be registered as Qualified Mental Health Professional (QMHP). Special Conditions:

  • Must obtain valid Driver's License within 30 days of employment to be eligible to operate a county vehicle
  • TB test (within 30 days of employment)
  • CPS (Child Protective Services) Check
  • Twelve-month probationary period
  • Work beyond normal work schedule, including on-call as needed for ACT Case Managers
  • Must be able to successfully pass CPR/First Aide training as well as behavior management as required by regulation, including but not limited to Title 12 of the Virginia Administrative Code. This training process includes a hands-on, in-person demonstration of first aid and CPR competency, for successful completion. This demonstration requires bending, lifting, and completing activities on the floor.

 


  The entry-level salary for this position is $49,409.00. Salary will commensurate based on years of experience and certifications, per the Hanover County career ladder.

Salary : $49,409 - $59,000
