Position Summary
We are seeking a caring and highly motivated individual to join our Home and Community-Based Support Team as a Community Residential Services (CRS) Support Specialist. This position plays a key role in providing direct support, teaching independent living skills, and offering advocacy for four individuals in a CRS setting.
The primary goal is to support residents in achieving housing stability, gaining personal independence, and building positive relationships within their community. You will help empower individuals to meet their personal goals in a supportive, respectful, and person-centered environment.
Key Responsibilities
- Provide individualized support in daily living activities (e.g., cooking, cleaning, hygiene, medication reminders)
- Teach and model independent living skills based on personalized support plans
- Encourage and foster healthy, respectful relationships in the home and the community
- Advocate for residents' rights, needs, and interests
- Assist with transportation and accompany individuals to appointments or community outings
- Document progress, incidents, and services per agency and state guidelines
- Work collaboratively with team members to maintain consistency and high-quality care
- Ensure a safe, clean, and welcoming home environment
Qualifications
- High school diploma or GED required; post-secondary education in Human Services or related field is a plus
- Experience working with individuals with disabilities or in a direct support role preferred
- Strong communication, problem-solving, and interpersonal skills
- Commitment to person-centered and culturally responsive care
- Ability to work independently and as part of a team
- Valid driver's license and reliable transportation required
- Must pass a DHS background check
