Job Summary:
We are seeking a compassionate and motivated Community Liaison / Care Coordinator to join our team. In this role, you will serve as the primary connection between clients, families, and our organization. The Community Liaison / Care Coordinator will be responsible for building strong community relationships, care services, and ensuring that clients receive the support they need to maintain their independence and quality of life.
The ideal candidate is professional, personable, and organized, with strong communication skills and a passion for helping others. You will play a key role in outreach, client intake, client care and ongoing coordination of services. making sure each client’s care plan is followed and adjusted as needed.
Responsibilities:
Community Liaison & Outreach
- Build and maintain relationships with referral sources such as hospitals, physicians, rehabilitation centers, senior organizations, and community agencies.
- Represent the company at community events, health fairs, and networking functions to promote services.
- Provide education to clients, families, and community partners about available home care programs and services.
- Act as the primary point of contact for new client inquiries and referrals.
In-Home Care Services
- Provide direct in-home care to clients as needed (such as personal care, companionship, light housekeeping, meal preparation, medication reminders, or mobility assistance, depending on training/certification).
- Support clients in maintaining independence and quality of life in their homes.
- Report changes in client condition to the care team promptly.
- Follow care plans and agency protocols while ensuring safety and dignity for clients.
Personal Care Assistant : Assist clients with activities of daily living, including bathing, dressing, grooming, and toileting.
Meal Preparation: Prepare and serve nutritious meals based on individual dietary needs and restrictions.
Medication Reminders: Ensure clients take their medications as prescribed and maintain accurate records.
Companionship: Engage in friendly conversation, leisure activities, and emotional support to foster a positive and comfortable environment for clients.
Mobility Assistance: Aid with mobility, transfers, and exercise as needed.
Housekeeping: Perform light housekeeping tasks, such as laundry, cleaning, and maintaining a safe and organized living space.
Transportation: Assist clients with transportation to medical appointments, errands, and other outings as required.
Communication: Maintain open communication with clients and their families, as well as with healthcare professionals and our care team.
