Job Summary
We are seeking a compassionate and dedicated CNA/Caregiver to join our home health care team. The ideal candidate will provide essential support to patients in various settings, including inpatient and outpatient care. Responsibilities
- Assist patients with daily living activities such as bathing, dressing, grooming, and eating.
- Monitor and record vital signs, including temperature, blood pressure, and pulse.
- Provide support for behavioral health needs, ensuring a safe and supportive environment.
- Help with mobility and transportation of patients within the facility or home community care settings.
- Maintain cleanliness and organization of patient areas.
- Document patient care activities accurately in health records.
- Communicate effectively with patients, families, and healthcare team members regarding patient needs and progress.
- Support individuals with disabilities by providing personalized assistance as required.
Experience
- Previous experience in a healthcare setting is required.
- Familiarity with geriatric care practices is advantageous.
- Knowledge of HIPAA regulations to ensure patient confidentiality.
- A background in behavioral health or home community care is a plus.
- Ability to work collaboratively within a multidisciplinary team while demonstrating empathy and patience.
