Position Overview:
Working on behalf of a family, provide education, instruction, direct services, and supports to empower and enable individual’s to achieve their stated goals and to live as optimally as possible. Facilitate community inclusion, integration and relationship building.

Role and Responsibilities:
- Meet the emotional, social, physical, and cognitive needs of each individual
- Ensure a person centered approach is used in all aspects of service delivery
- Ensure goals reflected in an individual’s plan are addressed and worked on with the individual
- Assist individuals in performing activities of daily living with as much independence as possible
- Assist in the development of social skills by providing education, training, and role
- Act as advocate for consumers
- Encourage self-advocacy and choices
- Prepare and maintain records of individual’s progress and services performed, reporting changes in an individual’s condition to supervisor
- Ensure safety and well-being of consumers at all times
- Ensure open communication with caregivers and management regarding any problems or issues that arise
- Performs other duties as assigned
