Per Diem Assessment nurse

Position Overview

We are seeking a dependable, detail-oriented, and compassionate Per Diem Assessment Nurse to conduct initial assessments and renewal assessments for clients. The ideal candidate will have strong clinical assessment skills, excellent documentation abilities, and the ability to work independently while ensuring quality care and compliance with applicable regulations and organizational standards.

Key Responsibilities

• Conduct initial client assessments to evaluate care needs, functional abilities, and clinical status.
• Complete renewal assessments to determine ongoing service needs and continued eligibility, as applicable.
• Accurately document assessment findings and submit required paperwork within established deadlines.
• Communicate professionally with clients, families, caregivers, and other members of the care team.
• Ensure assessments are completed in accordance with applicable regulations, policies, and procedures.
• Maintain confidentiality and protect client information.
• Identify changes in client condition or care needs and communicate concerns to the appropriate personnel.

Assessment Time Allocation

• Initial Assessments: 3 hours allotted per assessment.
• Renewal Assessments: 1.5 hours allotted per renewal assessment.

These allotted times include the assessment process and associated documentation, as applicable.
