Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
- Provide companionship and emotional support to clients.
- Monitor patients' vital signs and report any changes to the healthcare team.
- Prepare nutritious meals and assist with meal planning.
- Maintain a clean and safe environment for clients, including light housekeeping tasks.
- Administer medications as prescribed and ensure adherence to care plans.
- Support individuals with developmental disabilities, Alzheimer's care, dementia care, and memory care needs.
- Utilize equipment such as Hoyer lifts for safe patient transfers when necessary.
- Document patient observations and maintain confidentiality in accordance with HIPAA regulations.
- Collaborate with healthcare professionals to develop and implement effective care plans.
Skills
- First aid and CPR certification is highly desirable.
- Experience in home health, hospice care, nursing homes, or long-term care settings is preferred.
- Strong patient observation skills to identify changes in client conditions.
- Ability to perform heavy lifting when assisting clients with mobility needs.
- Experience working with individuals with disabilities or autism is a plus.
- Knowledge of cleaning procedures and maintaining a hygienic environment.
- Excellent communication skills to effectively interact with clients, families, and team members.
- Compassionate demeanor with a strong commitment to providing high-quality patient care.
