Case Assistance Worker
Ravenswood Older Adult Center – HANAC, Inc.
Location: Astoria, NY
Schedule: 28 hours per week
Monday–Thursday: 9:00 AM – 3:00 PM
Friday: 8:00 AM – 3:30 PM
Pay: $21.22 per hour (based on experience)
About the Role
HANAC is seeking a compassionate and organized Case Assistance Worker to support older adults by connecting them with benefits, services, and community resources. This position works closely with center participants to assess needs, provide referrals, maintain records, and assist with accessing programs that promote independence and well-being.
Key Responsibilities
- Conduct participant assessments and identify service needs.
- Provide referrals, follow-up services, and resource coordination.
- Assist with benefit, housing, and service applications.
- Maintain accurate case records and data entry in required systems.
- Conduct wellness calls and participant outreach.
- Collaborate with community agencies and service providers.
- Participate in staff meetings, training, and other program activities.
Qualifications
- Associate's Degree required; Bachelor's Degree preferred.
- Experience in social services, case management, or senior services preferred.
- Bilingual English/Spanish or English/Chinese preferred.
- Strong communication, organizational, and computer skills.
- Experience with Microsoft Office and data entry systems.
