Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including long-term care, assisted living, and acute care environments. As an LPN, you will play a crucial role in supporting residents ' health and well-being through comprehensive nursing practices.
Responsibilities
- Provide direct patient care, including administering medications and treatments as prescribed.
- Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
- Assist with IV infusion, catheterization, and other advanced nursing procedures.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Support patients with activities of daily living (ADLs) and promote their independence.
- Educate patients and families on health management, medication administration, and disease prevention.
- Participate in triage and emergency response as needed in various clinical settings.
- Ensure a safe environment for patients by adhering to infection control protocols.
Qualifications
- Valid LPN license in the state of WI.
- Experience in critical care, long-term care, or assisted living preferred.
- Familiarity with EMR/EHR systems such as ECP is advantageous.
- Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and CPT coding is beneficial.
- Strong skills in patient assessment, medication administration, and basic life support techniques.
- Experience working with diverse populations including seniors, individuals with developmental disabilities, and those requiring memory care or hospice services.
- Ability to perform procedures such as, specimen collection/processing, and physical examinations.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
