Current Illinois RN License

Education

  • CPR Certification
  • Crisis De-escalation Certification and/or related training strongly encouraged

Training And Experience

  • Experience in gerontological care required
  • Experience in acute-care clinical settings required
  • Psychiatric nursing experience preferred
  • Knowledge of community resources required
  • Microsoft knowledge/Computer proficiency required
  • Experience with Epic EMR preferred

Description

Hamilton Memorial Hospital District has an immediate opening for a part-time Secondary Registered Nurse at the Senior Enrichment Center. At HMHD our mission is to enhance the delivery of healthcare to our community and the surrounding area by providing accessible quality healthcare services.

Pay Scale: $22.22-$32.00 per hour

Job Summary

This position as a licensed registered nurse bears responsibility and accountability for the nursing care of patients on the Senior Enrichment unit. Inherent in the practice is the nursing process; the assessment of the patient status, the plan of nursing actions, the implementation of that plan, and the evaluation of the plan. The registered nurse assesses, and monitors patients’ health status and physical care needed at admission and while patients are present on the unit, including any physical/psychiatric emergencies. The registered nurse performs comprehensive assessments, takes physician’s orders, monitors/reconciles medications and provides health education for clients and families as needed or requested. They function as a member of the Multidisciplinary Treatment Team and perform administrative, clerical, and other tasks according to program policy. They demonstrate professionalism in all aspects of performance.

Physical Demands

The employee will be required to perform the following physical activities in the performance of their job duties:

  • Sitting/Standing/Walking/Bending Over/Kneeling/Crouching/Stooping/Balancing
  • Pushing or Pulling/ Twisting/Reaching Above Shoulder Level/Reaching Forward & Low,
  • Fine Finger Dexterity/Lifting or carrying (up to 25-lbs regularly)
  • Ability to hear ordinary conversation is required/Vision-Close and Distance-Color and Peripheral

Essential Functions

  • Plans and administers treatment in junction with Medical Director’s diagnosis and Plan of Treatment.
  • Keeps clear, concise written documentation, records of patient activities, care, treatment and progress.
  • Uses professional nursing principles, procedures and techniques in the care of psychiatric patients.
  • Communicates calmly with patients, family and visitors in situations that require tact and diplomacy.

Position Responsibilities

  • Performs comprehensive nursing admission assessment to obtain information regarding chief complaints, physical/surgical history, and review of all medications.
  • Updates & reviews patient medication list, noting all allergies and responses to medications.
  • Assist program RN in development of the nursing portions of the Multi-Disciplinary Treatment Plans, which includes documentation and implementation of identified nursing interventions.
  • Consults with Medical Director, Program Director, Therapists and other Healthcare providers on a regular basis to coordinate patient care.
  • Attends Treatment Team meetings to discuss and update Nursing Treatment Plan section of Multi-Disciplinary Treatment Plan and patient progress in treatment.
  • Initiates and conducts individual and group nursing education sessions, as needed or requested by program director and/or program RN.
  • Monitors patient consents.
  • Provides education on diagnosis and medications and assists patients in understanding of their diagnosis and the treatment process.
  • Communicates relevant clinical and other patient care information to Program Director, Medical Director and Treatment Team members as necessary and appropriate.
  • Monitors patients on an ongoing basis to identify health needs, problems, and/or symptoms.
  • Provides prompt, efficient, and responsive patient care.
  • Coordinates and documents patients’ care in cases of medical or psychiatric emergencies, providing a report to the Emergency Department, PCP, and/or other appropriate resource.
  • Completes nursing check-ins with patients present on unit each treatment day.
  • Monitors, observes and documents patient behaviors and symptoms.
  • Provides patient, family, and caregivers education on health-related topics.
  • Assists patients with personal care needs (bathroom, etc.) as needed or requested
  • Recognize your role as part of the QAPI efforts at HMHD.
  • Carry out QAPI roles and responsibilities as assigned.
  • Follow established policies and procedures in support of QAPI efforts.
  • Support the Just Culture concept and its use to improve patient safety.
