Hours: 8:30AM - 5PM, Weekdays
JOB SUMMARY
Coordinate the admission/transfer/discharge of patients in accordance with federal, state and local regulations, established procedural guidelines and as directed by your supervisor.
ESSENTIAL SKILL/KNOWLEDGE FUNCTIONS
- Participate in the pre-admission and admission process as well as arranging for
transfer of patients to other centers should the need arise.
- Obtain pertinent information from patients and/or family members, referral sources, and appropriate social agencies and verify financial information prior to admission and completes admission paperwork with family and patient.
- Verify that the patient room, etc. is ready prior to admission.
- Sustain contact and provide support to patient/families to include help in dealing with the familys feelings and financial responsibilities.
- Daily visits to new admissions to provide a smooth admission transition.
- Orient the patient to the center environment.
- Send follow-up thank you notes to families within one week of admission.
- Meet frequently with other social workers and discharge planners within the community.
- Maintain up-to-date information about agencies to which referrals may be made.
- Maintain active waiting list file.
- Communicate admission information to other departments.
- Complete understanding of admission packet and ability to explain to family/patient.
- Evaluate patient at hospital for appropriate placement as needed.
- Knowledge of long-term care admission requirements.
- Knowledge of advanced directives.
- Knowledge and understanding of Medicare, Medicaid, Private Pay and Third Party reimbursement.
ESSENTIAL ADMINISTRATIVE FUNCTIONS
- Participate in Nursing Center surveys (Licensure / JCAHO) and any subsequently required reports.
- Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.
- Attend and participate in mandatory in-services.
- Honor patients rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.
- Comply with Corporate Compliance Program.
- Report job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.
- Follow established safety regulations, to include fire protection/prevention, smoking regulations, infection control, etc.
- Follow established safety procedures when performing job tasks and/or working with equipment.
- Perform other related duties as necessary and as directed by supervisor.

JOB QUALIFICATIONS
LICENSURE/CERTIFICATION/EDUCATION REQUIREMENTS
Minimum of a high school diploma, or its equivalent.
Preferred Current RN or LPN license in good standing
OTHER TRAINING, SKILLS AND EXPERIENCE REQUIREMENTS
Customer service oriented. Should possess good marketing and communication skills. On-the-job training provided in admission guidelines. Computer skills preferred
SUMMARY OF OCCUPATIONAL EXPOSURES
Tasks assigned to position may involve potential and/or direct exposure to blood, body fluids, infectious disease, air contaminants and hazardous chemicals. May be subject to hostile and emotionally upset patients, family members, associates and visitors.
OTHER CONSIDERATIONS AND REQUIREMENTS
Should be able to communicate effectively, both verbally and in writing. The associate is required to lift a maximum of 5 to 10 pounds only. This position requires a considerable amount of walking. Associate must be able to drive a vehicle. Carrying is not frequently required as dollies are available. Associate should be able to sit and stand as tolerated.
