Hamden Rehabilitation & Health Care Center is a 153-bed skilled nursing and rehabilitation facility situated in Hamden, Connecticut. We specialize in short- and long-term care and have a secure memory care unit. We are located right in the heart of New Haven County, in close proximity to Yale New Haven Hospitals, yet conveniently only seconds from route 10, which runs throughout New Haven County.
Licensed Practical Nurses must have strong clinical skills, and previous experience in long term care preferred. Attention to detail, a positive attitude, good follow through skills and the ability to manage multiple tasks are required. RN/LPN's must work as a team, this critical to providing excellent clinical care.
Licensed Practical Nurses must demonstrate a willingness to assist others and work as a team member.
LPN Charge Nurse Responsibilities
- Licensed Practical Nurses are accountable for the medical care and well-being of the patients, including med-pass, treatments, order transcriptions, and thorough documentation.
- Licensed Practical Nurses coordinate, direct and oversee the delivery of care by ancillary staff.
- Licensed Practical Nurses demonstrate competence in clinical skills as well as electronic medical records.
- Licensed Practical Nurses collaborate with physicians and other health care professionals; comply with orders in a timely manner.
- LPN'S conduct unit rounds, ensuring that clinical practices are delivered within the facility's established standards of care.
- Licensed Practical Nurses maintain standards of care in accordance with regulatory agencies.
LPN Charge Nurse Qualifications
- Current State of Connecticut LPN License
- Licensed Practical Nurses must have excellent communication and interpersonal skills.
- LPN'S must have the ability to work as part of a team, while managing multiple tasks in a safe and effective manner.
- Licensed Practical Nurses must be courteous and professional with an exceptional "bedside manner."
- LPN'S must have a high tolerance for working under pressure and handling critical situations
