The LPN is responsible for primary care of the patient in collaboration with the provider. This nurse must be able to effectively communicate both in written and oral form. Excellent interpersonal communication skills. The nurse must be capable of prioritizing and organizing tasks related to optimal patient care. The nurse is expected to support the Center's mission or patient centered care and adhere to the Center policies and procedures, as well as applicable federal and state regulations. He/she is expected to demonstrate superior nursing skills and facilitation of nursing skills as part of the patient care experience. The position requires the ability to work flexible hours including some weekends and evenings.

REPRESENTATIVE DUTIES 

The following duties are typical for the LPN. Incumbents may not perform all of the listed duties and/or may be required to perform additional or different duties from those set forth below to address business needs and changing business practices.

  • Maintains Hawaii State Board of Nursing requirements and provides excellent Nursing care according to scope of practice, standards of practice and Hamakua Health Center’s policies and procedures.
  •  Demonstrates Nursing competency in all aspects of patient care. 
  • Demonstrates appropriate documentation and utilization of an electronic medical record to document care within nursing scope of practice.
  • Effectively prioritizes patient care and manages patient flow for maximum efficiency and optimum care provision. 
  • Consistently communicates accurate and pertinent information with patient care providers and other members of the care delivery team to facilitate effective and efficient patient care. Participates collaboratively and effectively with all members of the patient care team on an ongoing basis. 
  • Develops and maintains plan of care for patients that includes follow up on referrals to outside resources and health maintenance and prevention tracking.
  • Coordinates care with appropriate internal and external resources.
  • Consistently ensures patient/family satisfaction with nursing care. 
  • Applies appropriate age-specific/cultural considerations to plan of care on a consistent basis.
  • Provides competent on-site laboratory services with CLIA certification.
  • Facilitates process for quality specialty medical, diagnostic and therapeutic services to be available to patients via appropriate referral process and provides tracking for follow-up.
  • Actively participates in quality improvement and risk management programs at the Health Center. 
  • Participates in continuing education activities in order to maintain optimal skills and to develop new ones.
  • Participates in patient care equipment maintenance and quality monitoring. 
  • Demonstrates knowledge of emergency equipment and procedures.
  • Maintains medical instruments and is competent is autoclave procedures. 
  • Maintains Clinic supplies, medication room stocking, exam room stocking and elimination of expired medications and/or medical supplies.
  • Maintains a safe and clean working environment consistent with OSHA and HHC standards on a regular basis.
  • Recognizes and takes appropriate action on potentially unsafe acts and conditions.
  • Complies with all HIPAA regulations on a consistent basis. 
  • Utilizes universal precautions at all times.
  • Represents Hamakua Health Center positively in the community.

 EDUCATION, EXPERIENCE & QUALIFICATION GUIDELINES

 Any combination of education and experience that would likely provide the required knowledge and abilities is qualifying.  Current Hawaii State Practical Nurse License (LPN) required. • Current CPR Certification required. • Previous nursing experience in an ambulatory care setting preferred.

  • Demonstrated computer competency with multiple program interfaces with proficient computer data entry and typing skills. 
  • Effective written, oral and interpersonal communication skills. 
  • Demonstrate excellent teamwork capabilities. 
  • Demonstrated excellent organizational capabilities. 
  • Demonstrated aptitude and ability to learn multiple processes. 
  • Ability to multi-task effectively in a busy environment. 
  • Ability to provide excellent age and culturally appropriate customer service skills to internal and external customers consistently. 
  • Understand and support basic patient flow within the clinic.
