- Spend time in the field, assessing patients and enhancing referral relationships through hospital visits and community events.
- Educate guests and families on the benefits of Crest Healthcare's services, acting as a liaison during the transfer process.
- Collaborate with the DON and ADON to develop initial care plans and ensure smooth transitions for patients.
- Work with the Admissions Coordinator to provide daily updates on patient admissions and collaborate on reporting using internal and external data.
- Represent Crest Healthcare Group at community events and promote the mission of the organization.
- Previous experience as a Hospital Liaison with a strong network in Long-Term Care is preferred.
- Multi-facility experience is a plus!
- Strong communication skills, with the ability to engage diverse nursing and community care staff.
- Ability to achieve high levels of customer satisfaction and meet referral targets.
- Professional, dedicated, and knowledgeable about skilled nursing services and healthcare.
