The Location:

Hale Makua Health Services Home Health is a Medicare-certified provider of personalized care in the comfort of your home for individuals who are home-bound and need skilled help with medical conditions.

The Position

Under the general direction of the Clinical Manager, provides professional nursing care to patients following established

policies and procedures of the agency. Assumes overall responsibility for an assigned case load of home health patients,

providing assessment, coordination of ancillary services and community services, and may provide direct nursing health to

the patient in the patient's place of residence. May oversee and direct Licensed Practical Nurses (LPNs) Home Health

Aides (HHAs) in their day-to-day assignments. Answers questions from patients, responsible parties and other personnel.

Performs job duties in accordance with Ohana Pacific Health (OPH) Mission, Vision, Core Values and Core Behaviors.

Requirements

Education:

  • Bachelor of Science in Nursing (BSN) required.
  • Associate Degree in Nursing (ADN) may be considered in lieu of a BSN if the candidate has a minimum of six (6) years

of direct patient care experience in home health or a related clinical setting.

  • Graduate of an accredited school of nursing.

Licensure/ Certifications

  • Current Registered Nursing License in the State of Hawaii.
  • Current BLS certification.
  • Current medical/health screening including 2 step TB clearance.

Experience

  • Minimum six to twelve months recent experience in general medical/surgical nursing or other relevant work.
  • Knowledge of medical terminology and abbreviation required.
  • Knowledge of state and federal regulations governing home health services desired.

Required Skills, Knowledge, And Abilities

  • Good command of English, both written and verbal. Strong and effective communication skills.
  • Ability to ensure a high level of patient, family, employee, and external customer satisfaction.
  • Ability to collaborate with representatives from all areas of the Company.
  • Knowledge of medical terminology, electronic medical records, computer software systems and applications,

including but not limited to, time and attendance systems, Microsoft Office suite.

  • Ability to maintain accurate records. Demonstrate efficiency in Outcome and Information Set (OASIS) documentation.

Mental Requirements

  • Function independently, have flexibility, maturity, and personal integrity.
  • Ability to work with constant diversions and interruptions while paying attention to detail, to prioritize multiple and

sometimes conflicting assignments, projects, and requests.

  • Possess time management, reasoning, analytical thinking, and organizational skills.
  • Must deal tactfully with employees, patients, and families.

Physical Requirements

  • Is subject to injury from falls, burns from equipment, odors, dust, reactions to chemicals and air contaminates.
  • Must be able to move intermittently throughout the workday.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the

requirements of this position can be fully met.

  • Must meet the general health requirements set forth by the policies of the agency, which include a medical and

physical examination.

Other Requirements

  • Ability to work remotely and or tele-work as required.
  • Self-motivating; must understand and enforce safety and health requirements in patients' home setting.
  • May be requested to work from agency office from time to time.
  • Work hours may exceed 8 hours per day, include weekends, evenings, holidays.
  • Subject to call-back during emergencies.
  • May be involved in community activities.
  • Well-groomed professional appearance and demeanor.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must function independently in an uncontrolled environment (home setting), have flexibility, personal integrity, and

the ability to work effectively with other personnel.

Essential Duties

  • Demonstrates knowledge of the patient's diagnoses, limitations, expectations, plan of health, and current status.
  • Collaborates with Supervisor to coordinate all health for assigned patients.
  • Takes verbal/telephone orders from physicians and transcribes orders to patient chart, treatment/health plans as

appropriate. Follows-up on physician orders.

  • Administers treatments as ordered by the physician and according to policy and procedures.
  • Conducts accurate physical assessments of the patient, determines their skilled needs, and implements appropriate

interventions to meet those needs. Completes all paperwork by time sensitive deadlines.

  • Performs all aspects of case management.
  • Provides skilled care independently, within nursing scope of practice, including but not limited to central venous

access maintenance, wound/negative pressure wound therapy and tracheostomy care.

  • Apprises the family, physician, and others of changes/incidents.
  • Ensures infection control processes and procedures are followed by all staff in the home.
  • Orients new staff as assigned.
  • Collaborates with physicians, therapy services, consultants, and agencies to improve quality of services and to resolve

identified problems. Demonstrates strong customer service to patients, families, and others. Ensures family / patient

concerns are followed up promptly and informs Clinical Manager.

  • Makes recommendation for supplies, equipment, etc. to patient/family.
  • Adhere to visit schedule.
  • Performs minor housekeeping duties as needed. Maintains safety protocols and requires assigned staff to do the

same.

  • Participates in the survey process according to the procedures established by the agency.
  • Complies with all applicable statutes, rules, regulations, Conditions of Participation (CoP), policies and procedures,

standards of conduct.

  • Maintains confidentiality of patient information and the business of OPH.
  • Understands and models OPH mission, vision, and values.
  • Willingly performs other duties as assigned or requested.

Work Environment

  • Commute to and from patients' homes throughout the covered geographic area.
  • Travel to patients' homes, nursing care facilities, and other sites as needed.
  • May be in close contact with individuals who have infectious diseases.
