Summary of Position:
Works closely with the healthcare team in accomplishing the mission of Hale Ho Aloha and providing optimal care to our residents. The Physical Therapist Assistant will plan, organize, and assist with a our resident's functional maintenance exercise program in accordance with established policies and procedures, and standards of practice as delineated by Hale Ho Aloha. In addition, may perform the delivery of direct physical therapy services to patients, under the supervision of a Physical Therapist.
Essential Duties:
- Develops a resident’s functional maintenance exercise program with the intention of maintaining the resident’s current functional abilities as it relates to physical function and completion of activities of daily living (ADLs).
- Assists residents in completing functional maintenance exercise program. May modify a resident’s program in accordance with changes in patient status within the scope of the overall functional maintenance program goals.
- Complete documentation of daily program participation and progress, per company guidelines
- Provides clinical direction, including instruction and delegation of tasks to restorative nurse aides as it relates to a resident’s functional maintenance exercise program.
- Actively participates with planned resident care plan meetings as a member of the interdisciplinary team and completes required documentation.
- Assists the company Physical Therapists in the provision of direct patient care per physician orders and within the scope and limitations set forth by the statues and/or rules of the profession.
- Upholds the values of Hale Ho Aloha, ensuring that the mission and vision of the company are understood and accomplished with consistency.
Requirements:
- Graduate of an accredited Physical Therapist Assistant program
- Currently State of Hawaii Physical Therapist Assistant License
- Current First Aid and CPR (BLS) Certification
- State of Hawaii TB clearance
- Physical Exam (within 1 year)
- 2 References Required
- Experience in skilled nursing or long-term care preferred
- Excellent written, verbal, non-verbal, and active listening communication skills
