What are the responsibilities and job description for the Direct Care Worker (DCW) position at Haddon Home Care?
Job Summary:
The DCW provides personal care and related services in the home. She/he functions under the direction, instruction and supervision of the staff nurse and the Director of Nursing and/or appropriate supervisor.
Organizational Relationship: The DCW reports directly to the General Manager.
Occupational Exposure Category: A: High exposure to blood borne pathogens
Qualifications:
Completion of the 9th grade; prefer high school graduate
Ability to read and write in English, consistent with job requirements
Completion of skills competency.
A reliable means of transportation to get to assigned client visits.
Prefer 1 year experience as a DCW in a health-related setting.
Responsibilities:
Duties of the DCW may include:
- To assist with ADLS and ADLs as per the POC
- To follow the POC for each client and document the same
- Helps the client to maintain good personal hygiene
- Assists in maintaining a healthful, safe environment
- Plans and prepares nutritious meals. Markets when indicated on the POC
- Assists the client with ambulation as per the POC
- Assists the therapy personnel as needed with rehabilitative processes
- May cue for medications ordinarily self-administered as assigned
- Taking and charting vital signs as per the care plan
- Encourages the client to become as independent as possible according to the nursing care plan
- Attempts to promote client's mental alertness through involvement in activities
- Gives simple emotional and psychological support to the client and other members of the household
- Establishes a relationship with client and family which transmits trust and confidentiality
- Prepares a report of visit on the day it is performed and incorporates same in the clinical record weekly or as directed
- Reports any change in the client's status or home situation to immediate supervisor, or RN
- Carries out their assignment as instructed by the nurse
- Works with personnel of other community agencies involved in the client's care as directed by the staff nurse
- Performs routine housekeeping tasks as related to a safe and comfortable environment for the client, as instructed by the professional nurse
- Attends in-service as required by state and federal regulations
- Notifying Haddon Home Care of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, which will affect agreed service with Haddon Home Care.
- Wears Haddon Home Care's name badge at all times they are seeing Haddon Home Care clients.
Functional Abilities:
Must be able to read 12 point or larger type.
Must be able to hear and effectively communicate in English.
Must be able to stoop and bend.
Must be able to travel to prospective clients' residences.
Must be able to lift, turn, and transfer clients weighing up to 200 pounds
Must be able to carry bundles weighing up to 10 pounds upstairs.
