- Serve as an active member of the care management team, providing leadership and clinical guidance
- Deliver comprehensive primary care within scope of PA/NP training
- Coordinate and support preventive care, diagnosis, treatment, and follow-up services
- Communicate patient care plans effectively with team members
- Prescribe medications as appropriate
- Maintain accurate and timely documentation of diagnoses, treatments, medications, and consultations
- Graduate of an accredited Physician Assistant or Nurse Practitioner program
- Active and unrestricted PA or NP license in the State of Michigan
- Current NCCPA certification (for PAs) and BLS/ACLS certifications
- Eligible for liability insurance coverage
- Demonstrated commitment to serving underserved populations
