What are the responsibilities and job description for the QIDP/ CASE MANAGER position at Habilitative Systems Inc?
SUMMARY
Ensures that clients receive the services outlined in the Individual Habilitative Systems service plan in accordance with the policies of the agency, funder, licensing and accreditation entities. Perform various case management tasks and implement person centered plan for positive impact on the life of each individual.
QUALIFICATIONS
- Bachelor’s Degree in special education rehabilitation or a related human service field from an accredited college.
- One (1) year QIDP experience required.
- Current CPR certification.
RESPONSIBILITIES
- Work cooperatively with other department counselors and instructors, referral/sources, parents, guardians to maximize to client services.
- Document the Individual Service Plan (ISP) for individuals receiving services made by medical and professional consultants.
- Communicate with parents/guardians, peers, staff, and other community members regularly to maintain positive relationships and to ensure that service and support are implemented and continue to meet the individuals desired outcomes and needs.
- Seek opportunities for community engagement and applicable external services based on client’s individual needs and in accordance to the individual service plan.
- Provide staff in-service training.
