Enroll patients over the phone for one of the nation's leading Chronic Care Management providers.

H3C provides Chronic Care Management for many clinics nationwide. We offer telephonic solutions to our clients to provide outreach on their behalf. We reach out to thousands of Medicare patients to help them manage their health conditions on a monthly basis. H3C uses cutting-edge technology to deliver quality care to the patients and clinics we serve.
The Position:
We are looking for Enrollment Specialists to call patients to explain what the Chronic Care Management program has to offer them and obtain patient consent for enrollment into the program. The ideal Enrollment Specialist has call center experience with emphasis on the sale of a service.
