LPN Field Nurse

H2 (H-Squared) Homecare is looking for compassionate, energetic Licensed Practical Nurses (LPN) to provide exceptional care for our pediatric patients in their homes. 

We are looking for nurses with the ability to work under minimal assistance and are committed to providing the most excellent care for our patients in the comfort of their home and school settings. 

We currently hiring for full-time, part-time, and PRN opportunities throughout the state of Georgia.  

Credentials:

• Currently licensed as a licensed Practical Nurse or Licensed Vocational Nurse in the State of Georgia
• Graduate of an accredited practical nurse or vocational nursing program
• One year prior professional nursing experience, homecare experience preferred
• Current CPR certification
• Clean background check
• Covid-19 vaccination is not required

Job responsibilities:

• Changes in patient’s condition are identified and documented with resolution on appropriate clinical form
• Physician notification to obtain necessary orders for intervention(s) per state regulations
• Performs specific treatments and medication administration in accordance with physician orders
• Documentation reflects patient collaboration and participation in their plan of care
• Documentation and clocking in and out utilizing the current patient health record platform Alayacare
• Skilled observations and significant changes in patient status are communicated to the Director of Clinical Services and/or Clinical Supervisor/Case Manager
• Request for modification of orders and additional orders are made based on the patient’s clinical status and Skilled nursing judgment
• All physician collaboration /coordination of care is documented in the medical record and communicated as needed
• Adheres to H-squared Homecare policies and procedures
