Summary
Join Moffitt Cancer Center as an Ambulatory Nurse Case Manager, a vital member of our multidisciplinary care team dedicated to delivering high-quality, patient-centered care in an outpatient setting. In this role, you will coordinate and monitor care across the continuum, focusing on improving patient outcomes, reducing avoidable hospitalizations, and ensuring timely access to services.
Additionally, the Ambulatory Nurse Case Manager promotes effective utilization and monitoring of health care resources by collaborating with the health care team, maintaining accurate knowledge of federal, state, and payer standards, and acts as a patient advocate by ensuring timely access to services.
You will serve as a patient advocate and resource navigator, collaborating closely with physicians, advanced practice providers, social workers, and other healthcare professionals. Your responsibilities will include arranging home health services, durable medical equipment, and infusion needs, while maintaining compliance with CMS, Joint Commission, and institutional standards. Strong communication skills, clinical judgment, and a commitment to excellence in care coordination are essential.
Minimum Experience Required
- Graduate of an CCNE or ACEN accredited school of nursing required
- Current Florida RN licensure Required
- BLS Certification
- Minimum 5 years of clinical experience in an acute care setting required
- Minimum 1 year of case management experience in an acute care setting required
- Must be able to work independently with minimal direction, anticipate and organize workflow, prioritize and follow through on responsibilities
Preferred Experience
- BSN Preferred
- ACM or CCM Certification
- Demonstrated experience in oncology nursing
- Extensive knowledge of nursing care, healthcare trends, community and healthcare resources
- Knowledge of state and federal regulations governing discharge planning and other regulatory agency requirements by the Joint Commission and CMS
- Strong interpersonal communication skills
- Competent computer skills to include EHR systems and Microsoft office products
