Position Summary
The Case Manager is responsible for coordinating and monitoring services for individuals receiving substance abuse treatment. This role focuses on care coordination, advocacy, documentation, and linkage to community resources while supporting clients in achieving treatment goals, stability, and recovery. The Case Manager works collaboratively with clinical staff, medical providers, and external agencies to ensure continuity of care.
Key Responsibilities
Client Care & Coordination
- Conduct intake screenings and assist with admissions and discharges
- Complete biopsychosocial assessments and ongoing case management assessments
- Develop, implement, and monitor individualized service plans in collaboration with the treatment team
- Coordinate referrals to medical, mental health, housing, employment, legal, and social service resources
- Assist clients with appointments, transportation planning, and follow-up services
- Advocate for clients’ needs within the treatment program and the community
Documentation & Compliance
- Maintain accurate, timely, and compliant client records
- Complete progress notes, service plans, referrals, and discharge summaries
- Ensure documentation meets state licensing, accreditation, and funding requirements
- Maintain confidentiality in accordance with HIPAA and 42 CFR Part 2
- Participate in audits, chart reviews, and quality assurance activities
Collaboration & Communication
- Participate in interdisciplinary team meetings and case conferences
- Communicate effectively with counselors, peers, nurses, and program leadership
- Coordinate with probation, courts, child services, and other external agencies as needed
- Provide updates to referral sources and collateral contacts (with proper releases)
Client Support & Education
- Assist clients with goal setting and problem-solving
- Provide education on recovery resources, community supports, and life skills
- Support relapse prevention planning and aftercare coordination
- Encourage engagement in treatment, groups, and recovery activities
QualificationsRequired
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field
- Experience working in substance abuse treatment, behavioral health, or social services
- Knowledge of substance use disorders, recovery principles, and community resources
- Strong organizational, communication, and documentation skills
- Ability to maintain professional boundaries and ethical standards
Preferred
- Case Management certification or relevant licensure (if applicable by state)
- Experience with residential treatment, MAT programs, or co-occurring disorders
- Familiarity with state regulatory and licensing requirements
