We are seeking a dynamic and compassionate Case Manager to join our dedicated team. GENERAL STATEMENT OF RESPONSIBILITIES: Responsible for the direct oversight of

cases, including reports using the Structured Decision Making (SDM) process via MiSACWIS,

Child Welfare Licensing (CWL) expectations to maintain placements; attends all court hearings and home visits.

· Maintains proper documentation in legal files for all assigned cases.

· Submits (by established deadlines) all reports, Initial Service Plans, Updated Service Plans, SDM paperwork, court reports, and agency specific paperwork on time. All case information must be input and/or downloaded via Michigan Statewide Automated Child Welfare Information System (MiSACWIS).

· Develop personalized care plans that outline goals and connect clients with appropriate resources such as ILP or Aftercare programs.

· Manages a schedule to adequately reflect parenting time, case conferences, court hearings, etc.

· Developing individualized case plans that address barriers to independence and promote successful transition.

· Providing on-going monitoring and evaluation to ensure participants are progressing toward their goals.

· Connecting participants with community resources and services to support their continued success.

· Providing individual and group counseling sessions to support participants' emotional, behavioral, and psychological well-being.

· Conducting assessments to identify areas where participants may require additional support or intervention.

· Collaborating with internal and external partners to ensure a comprehensive approach to supporting program participants.

· Oversees home provider compliance during monthly home visits, tracking essential CWL materials (medicals, dentals, educational, and compliance with statutes).

· Providing intensive therapeutic counseling services to program participants.

· Attends IEP meetings and other educational meetings, as outlined in respective case plans.

· Provides staff support to the program.

· Maintains contact with each assigned Michigan Department of Health and Human Service (MDHHS) worker, ensuring documentation is signed and placed in the respective case files. This includes case planning, permanency planning, and discharge planning.

· Independent living services will be provided to each youth being served, obtaining vital records such as birth certificate and driver’s license.

· Prepares and presents timely updates, reports and other necessary communications to the Program Manager.

· Plans, administers and prepares a variety of narrative and statistical reports.

· Manage confidential information.

· Regular and consistent attendance.

· Other duties as assigned.

Required

· A bachelor’s degree in a human Behavioral Science, Criminal Justice, or related field from an accredited college or university, and 3-4 years of experience as a case manager.

· A background in human services, job development, criminal justice, or crisis intervention experience working with youth, or related field, in youth development services. *Master’s degree preferred.

· Must be MDHHS trained as a foster care case manager

· Must be MDHHS trained as a foster care supervisor

· Must have working knowledge of MiSACWIS system

· Must be organized, self-directed, pay attention to detail, and deadline oriented.

· Ability to understand and follow complex oral and written directions.

· Ability to identify needs and provide interventions, training, guidance, and support in implementing treatment and service plans for children.

· Ability to write concise, accurate and informative reports.

· Ability to think and act quickly and appropriately in emergencies.

· Must possess a valid State of Michigan Driver’s License, have a safe driving record. Must be maintained throughout employment.

· Must pass a State Police Background Check and Fingerprinting.

· Negative TB Test.

· MDHHS Central Registry Clearance.

· Social Work Licensee.

· Must be CPR and First Aid certified (Within 90 Days of Onboarding).

Skills

  • Strong expertise in crisis intervention and crisis management strategies that promote immediate safety and long-term stability.
  • Proven experience in addiction counseling, behavioral health support, or social work with diverse populations.
  • Knowledge of child welfare systems, SIL and IL programs, Medicaid benefits, Foster Care, State Laws, MDHHS systems, and workforce development initiatives.
  • Exceptional communication skills with the ability to motivate clients through motivational interviewing techniques.
  • Supervisory experience demonstrating leadership in mentoring staff or interns while maintaining high-quality service standards.
  • Ability to handle sensitive situations involving childcare needs or mental health crises with professionalism and empathy.
  • Familiarity with community resources related to Medicaid services, behavioral health treatment options, and social support networks. Job Type: Contract

Pay: $18.00 - $23.00 per hour

Expected hours: 5 – 10 per week
