Gut Feelings Psychosomatic Medicine is a specialty mental health practice focused on the gut–brain connection and the ways stress and trauma can present as both emotional and physical symptoms. We deliver evidence-based psychiatric care and interventional services, including psychiatric evaluations, medication management, mind–body strategies, Transcranial Magnetic Stimulation (TMS), MeRT, Stellate Ganglion Block Sympathetic Reset, and Spravato (esketamine) for eligible patients. Our model is patient-centered and highly collaborative, partnering with patients and medical teams to provide comprehensive assessment, responsive communication, and holistic care.
We’re hiring an Oakland-based PMHNP to provide high-quality telehealth psychiatric medication management for an adult outpatient population, with occasional in-person shifts to support and/or supervise interventional psychiatry services. This role is ideal for a clinician who enjoys thoughtful longitudinal care, is strong with documentation and follow-through, and is also comfortable with infusion workflows—including peripheral IV insertions.
Most clinical hours will be telehealth med management. In-person work is scheduled periodically based on patient needs and interventional programming (e.g., infusion days, treatment monitoring, clinical coverage).
Key ResponsibilitiesTelehealth Medication Management (Primary)- Conduct psychiatric evaluations and ongoing medication management follow-ups via telehealth.
- Diagnose and treat common outpatient psychiatric conditions (e.g., depression, anxiety, ADHD, PTSD, bipolar spectrum, insomnia), including complex comorbidity where clinically appropriate.
- Prescribe and monitor psychiatric medications, including controlled substances when indicated and consistent with applicable regulations and clinic protocols.
- Order and interpret labs and other diagnostics as clinically appropriate; coordinate care with therapists, PCPs, and other specialists.
- Provide patient education, shared decision-making, and structured follow-up plans.
- Perform and/or supervise interventional treatment visits in the Oakland clinic as scheduled.
- Start peripheral IVs and manage basic infusion set-up as needed.
- Monitor patients during treatments, including vitals, side effects, and clinical response; document appropriately.
- Respond to and triage treatment-related issues and escalate to the supervising physician when indicated.
- Support safe clinical operations: adherence to workflows, checklists, and patient monitoring standards.
- Collaborate closely with the medical director/physician team and clinical staff to maintain consistent, high-quality care.
- Communicate with patients and staff promptly and professionally.
- Maintain timely, accurate documentation and coding/billing support consistent with clinic expectations.
- PMHNP (Psychiatric Mental Health Nurse Practitioner) with active California NP licensure (in good standing).
- Active DEA registration (or ability to obtain promptly) and CA furnishing requirements as applicable.
- Comfortable providing telehealth psychiatric medication management with strong clinical judgment and risk assessment skills.
- Comfort with IV infusions and peripheral IV insertion (hands-on competency required).
- Strong documentation practices and ability to work efficiently in an EHR.
- Ability to reliably attend occasional in-person shifts in Oakland for interventional days/coverage.
- Prior experience with interventional psychiatry (e.g., ketamine/esketamine programs, infusion clinic workflows, procedural monitoring).
- BLS required; ACLS strongly preferred (or willingness to obtain).
- Experience working with treatment-resistant depression and higher-acuity outpatient populations.
- Comfort collaborating in a measurement-based care environment and using standardized rating scales.
- Hybrid work: primarily remote/telehealth with periodic in-person clinical days in Oakland.
- Supportive physician collaboration and clear clinical escalation pathways.
- Operational support, including clinic systems and tools to improve efficiency (e.g., optional documentation support/AI-augmented scribing tools).

This position is designed for a local Oakland-based clinician. Telehealth may be performed remotely (within approved locations), but the role requires the ability to be onsite in Oakland on a scheduled basis for interventional treatment days and occasional clinical needs.
