As an Instructor, you would be responsible for conducting the Home Health Aide Training Programs in compliance with NYS DOH Regulations and participation with Inservice Training.

Education And Experience Requirements
- A current New York State Nursing License as a Registered Nurse.
- Previous nursing experience in home health care is required.
- Previous experience as a Nursing Instructor for Home Health Aide Training Programs.
- Prior teaching experience and familiarity with electronic medical record applications a plus.
- Excellent teaching skills, ability to relate to HHA staff, patient assessment skills and the ability to efficiently manage work schedule are required.
- Comfortable with a laptop/tablet and integrating them into a home visit.
- Ability to travel to patients' homes to make home visits.
- Responsible for evaluating, planning, and coordinating, health care to patients who are admitted to the LHCSA Program.
- Works proactively with the Clinical Manager to ensure ongoing effectiveness/compliance of the HHATP.
- Assists with the management of the curricular practical components of the Home Health Aide training program.
- Conducts training classes/competencies.
- Knowledge and understanding of the care requirements of the adult and geriatric population
- Able to recognize when a patient is in pain and take appropriate actions.
- Coordinate services to ensure that patient's needs are met.
- Re-evaluate needs and modify plans as indicated.
- Establish realistic goals and methods for goal achievement with the patient/family and other care-givers through a coordinated patient care plan.
- Responsible for the delivery of direct nursing services and its documentation.
- Maintain complete and current records reflecting all aspects of the patients care.
- Supervise home health aide (HHA) services through periodic visits to patients' homes including the assessment of care delivery according to the nursing care plan and an evaluation of the workers competence
- Notifies the patient's physician of any change in the patient's condition.
