Intake Specialist

Set primarily on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, Gurwin's Assisted Living community (Gurwin Jewish : Fay J. Lindner Residences), our independent living community (Fountaingate Gardens), Island Nursing and Rehab Center, and two certified (CHHA) and licensed home care (LHCSA) agencies.

EDUCATION AND EXPERIENCE REQUIREMENTS:

• College degree preferred.
• Minimum of 2 years of home care or related office experience.
• Excellent interpersonal and organizational skills required.
  
  
  

JOB RESPONSIBILITIES:

• Actively monitor the intake referral system throughout the day for potential clients.
• Assists in marketing efforts for potential clients. Go back to discharged patients and make follow-up calls to CHHA patients.
• Effectively communicate with the patients, family members, physicians, discharge planners from hospitals and staff.
• Check eligibility of potential CHHA patients on various electronic platforms.
• Data enters new/maintain patient demographics and insurance information into the Crescendo (AlayaCare) System.
• Upload and file patient referral documents, clinicals, MARS, order summary, progress notes, H&P, medication list, hospital discharge summary and face to face.
• Run and send monthly reports to the management.
• Answers telephone for LHCSA and CHHA, records and relays all messages, prepares call slips when indicated, including on-call scheduling and updates with answering service.
• Demonstrates an attitude of cooperation and professionalism when dealing with peers and supervisors. Performs work in the true spirit of the team.
• Performs job function with maximum efficiency and effectiveness, consistently aware of the fiscal impact of actions.
• Successfully meets targeted performance indicators related to clerical position and performs other tasks as assigned.
• Accepts direction and constructive feedback, comprehends overall concepts communicated and makes necessary changes to improve results.
• Consistently performs responsibilities with focus to customer service. Understands who the internal and external customer is, addresses issues, and resolves through to full recovery.
• Maintains appropriate level of confidentiality consistent with HIPAA regulations. Adheres to the compliance policy and procedures. Demonstrates appropriate and professional behavior, represents the agency in a professional and courteous manner.
