Gundersen Health System (becoming Emplify Health) is seeking a full-time skilled Social Worker for our Hospice team who is committed to excellence, enjoys teamwork, and contributes daily to our mission, vision and values.
What You Will Work
- This position is full-time (72 hours bi-weekly), primary day shifts Monday -Friday 8am - 4:30pm
- Every 5th weekend on-call. There is no call during the week or weekends after 4pm
- Holidays would be approx. 1 holiday per year on-call only
- Provide assessment, planning, consulting, education and referral services to Gundersen Health System patients and family members
- Act as consultants and leaders in comprehensive care throughout the system, while supporting organizational strategies and goals
- Pay starting at $25.40/hour more for relatable experience

- Bachelor's degree in Social Work from an institution accredited by the council on Social Work Education OR
- Bachelor's degree in psychology, sociology, or other field related to social work and is supervised by an MSW OR
- Bachelor's degree from a school of social work accredited by the Council on Social Work Education, is employed by the hospice before December 2, 2008, and is not required to be supervised by an MSW.
- 1 year of social worker experience in a health care setting
- Social Worker license in state of practice
