GENERAL SUMMARY:
The Service Plan Manager is responsible for coordinating and overseeing the development, implementation, and evaluation of individualized service plans for youth residing in the residential program. This position ensures that each youth’s program is goal-oriented, integrated, and consistent with best practices and licensing standards. The Service Plan Manager promotes the personal growth, safety, and dignity of youth while ensuring family and community collaboration to support successful outcomes and transitions to the least restrictive environment.
ESSENTIAL JOB FUNCTIONS:
1. Service Planning and Implementation
- Coordinate and oversee the implementation of each youth’s individualized service plan (ISP).
- Integrate all aspects of the youth’s program, including education, therapy, behavior management, and daily living activities.
- Monitor and document progress using objective indicators; make timely modifications as needed to ensure continued growth.
2. Program Evaluation and Documentation
- Conduct quarterly reviews of each youth’s service plan, assessing outcomes, identifying barriers, and recommending program adjustments.
- Review, sign, and date all service plan documents and progress reports to verify accuracy and compliance.
- Maintain up-to-date records consistent with agency policy, licensing requirements, and confidentiality laws.
3. Monitoring and Advocacy
- Ensure each youth receives a 30-day review of their continued need for residential placement.
- Advocate for timely step-down or discharge to less restrictive settings when appropriate.
- Monitor and document any use of restraint, special restrictions, or behavior management plans to ensure compliance with policy and youth rights.
- Promote trauma-informed and strength-based practices across all services.
4. Family and Community Engagement
- Serve as the primary liaison between the youth, family, program staff, and external service providers.
- Support families in understanding and participating in their child’s treatment and service planning process.
- Assist staff in understanding family dynamics, social influences, and behavioral factors that impact youth adjustment.
- Facilitate referrals and coordinate with community agencies to ensure continuity of care and successful reintegration into home or community settings.
5. Rights Protection and Professional Conduct
- Uphold and advocate for the human and civil rights of all youth and their families.
- Foster a program culture that promotes respect, self-worth, and empowerment.
- Participate in multidisciplinary team meetings and staff trainings as required.
- Maintain professional boundaries and adhere to all ethical and confidentiality standards.
6. Other
- Adhere to all federal and state laws and standards pertaining to the operation of the agency
- Assist in addressing areas on non-compliance identified by licensing inspections and complaint inspections
- Assist in delegating appropriate responsibilities to other staff, including the responsibility of overseeing the facility during absence.

- Assist in facility management, including maintenance
- Assist in establishing effective communication between staff and residents
- Assist in reporting injuries, deaths, and critical incidents involving residents to the appropriate authorities

Pay: $44,750.00 - $45,500.00 per year
