Overview
We are seeking compassionate and dedicated Direct Care Workers to provide essential support to individuals in need of assistance with daily living activities. To provide PRN (as needed) "companionship services" for the care, fellowship and protection of persons who, because of advanced age or physical or mental infirmity, cannot care for themselves and to help them reach their maximum potential. PRN employees are not assigned to a regular shift and serve as back-up for other employees who cannot work an assigned shift. As such, PRN employees must be reliable back-ups and available on rather short notice. Parishes services by the Houma Region include Terrebonne, Lafourche, St. Charles, St. John the Baptist, St. James, St. Mary and Assumption.
Essential Job Functions:
- Provide direct care "companionship services: as needed. Services may include assistance with grooming, bathing, dressing, meal preparation, feeding, toileting, cloths washing, diaper changing, assisting with ambulation, providing client transportation for shopping/appointments, leisure/community activities and other similar personal care services at the time and place defined in each client's Individual Service Plan.
- Monitor your client(s) during each assigned duty shift, as required by the client's Individual Service Plan. Accurately complete all required documentation, including on a daily basis your client's documentation of services provided as events occur, and the status of client(s) to be reported on progress notes/service logs.
- Perform all job duties in accordance with prescribed safe work practices, including the use of closed-toe, non-slip shoes, using proper body mechanics and/or lifting devices and the consistent practice of universal precautions.
- Perform non-complex medical task ask prescribed by the clients Individual Service Plan only when trained to do so by medical personnel. Non-complex tasks may include administering medication when delegated by authorized medical personnel.
Knowledge, Skills and Abilities:
- When required to transport clients; applicants must have a valid Louisiana driver's license and registration, Louisiana automobile liability insurance and access to a reliable vehicle in safe operations condition with a current Louisiana inspection sticker.
- Anyone transporting clients must be at least 23 years of age with two years of continuous driving experience per agency insurance and licensing requirements.
- Have the ability to read and write at a level that ensures understanding of the client's service plan, ability to document services provided and carry out directions competently as assigned.
- Must have the qualities of patience, empathy, dependability and the ability to teach others.
- The ability to bend, stoop, sit, stand, reach and lift as designated by a client's individual service plan. Possess the physical stamina required to help individuals who need assistance with ambulation and the emotional strength necessary to work with individuals who need personal care services.
