Gulf Coast Medical Center is seeking an experienced Physician Assistant (PA) to join our well-established, multi-specialty practice serving West Pasco and Hernando County for over 44 years.

What You’ll Do
- Provide comprehensive primary care (evaluations, diagnosis, treatment plans)
- Manage chronic conditions (Diabetes, COPD, CHF, CKD, Hypertension)
- Perform Annual Wellness Visits (AWVs), preventive care, and hospital follow-ups
- Support Chronic Care Management (CCM) and Transitional Care Management (TCM)
- Close care gaps and support quality initiatives (MIPS, HEDIS)
- Perform medication reconciliation and patient education
- Collaborate with physicians, nurses, and care coordination team
- Document in EMR (eClinicalWorks preferred)
What We’re Looking For
- Active Florida PA license
- Accredited Physician Assistant program graduate
- DEA license (or ability to obtain)
- 2 years of primary care experience preferred
- Strong communication and teamwork skills
- Experience with value-based care or quality programs preferred
- Bilingual (English/Spanish) a plus
