ABOUT GUIDING LIGHT HOSPICE
Guiding Light Hospice is recognized for clinical excellence, family satisfaction, and a standard of care that goes beyond the benchmark. CHAP accreditation means we've been independently verified to meet the highest national standards in hospice care. We don't just meet the standard. We set it.
We are intentional about the team we build. Our culture values accountability, professionalism, and meaningful impact. Leadership is accessible. Standards are high. Care is personal.
ABOUT THIS ROLE
We're looking for a compassionate community liaison who builds relationships rooted in trust — someone who listens first and genuinely understands the needs of patients, families, and care partners before anything else.
This role is about education, connection, and advocacy. You'll work alongside physicians, social workers, case managers, and community organizations to ensure families know all the comfort-centered options available to them — long before a crisis moment arrives.
WHAT YOU'LL DO
- Build lasting relationships with hospitals, skilled nursing facilities, and physicians to become a trusted resource in your community
- Educate families and care partners about hospice benefits and comfort care options with honesty and empathy
- Collaborate closely with clinical staff to ensure seamless, coordinated care for every family we serve
- Represent Guiding Light Hospice with professionalism, warmth, and a deep commitment to our mission
QUALIFICATIONS
- 3 years of experience in hospice or home health, with a demonstrated ability to build trust-based relationships across care settings
- Deep understanding of hospice eligibility, benefits, and the Medicare hospice benefit
- Proven ability to work collaboratively with clinical and interdisciplinary teams
- Strong communication skills — comfortable speaking with families, physicians, and facility staff
- Self-motivated and organized with the ability to manage a territory independently
WHO YOU ARE
You feel called to end-of-life care — not just drawn to it professionally, but personally. You communicate with warmth and clarity, hold yourself to a high standard, and find deep fulfillment in making sure families never feel alone in the hardest moments of their lives.
If you are a hospice professional who holds yourself to a higher clinical standard, you'll feel at home here.
