What are the responsibilities and job description for the CLINICAL CARE COORDINATOR position at Guardian Recovery?
POSITION PURPOSE:
The Care Coordinator is responsible to ensure a safe, comfortable, and supportive environment for the clients participating in the program.
Qualifications:POSITION REQUIREMENTS/QUALIFICATIONS
EDUCATION:
Preferred Bachelor’s Degree from accredited university or equivalent. Minimum requirement of High School Diploma or equivalent. Experience may substitute for a degree.
EXPERIENCE:
At least six months’ experience working in the substance use or mental health field.
LICENSURE/CERTIFICATION:
CPR certification preferred.
KNOWLEDGE AND TRAINING REQUIRED AT TIME OF HIRE:
Knowledge of the Principles and Practices of the addictions and mental health field.
Knowledge of Joint Commission Standards.
Demonstrates Proficiency in Communication & Written skills.
Knowledge of State & Federal Statutes Regarding Client Confidentiality laws.
Knowledge of Drug-Free Workplace Policies.
Knowledge of Workplace Violence Polices.
Knowledge of addictions and mental health complications.
Knowledge of community healthy resources that support life improvement in physical, emotional, mental and spiritual health, economic security, employment, positive social and family interdependence and positive cultural identity including:
- Referral procedures and admittance criteria for health care and community providers.
- Ability to network and collaborate with healthcare and other social and cultural agencies that can provide services to the clients and families.
Knowledge of the 12-Step Recovery Program.
Knowledge and competency in problem solving, stress management, ethics and team building.
Knowledge of alcohol and other drugs that includes:
- Alcohol and addictive drugs and their physical, emotional, intellectual, and spiritual impact on the individual.
- Impact of addiction on the family.
- Ability to determine if a medical emergency exists and to take appropriate action, when necessary.
- Knowledge of emergency procedures used in case of alcohol and/or drug overdose.
- Knowledge of the stages of alcohol and other drug withdrawals and ability to take appropriate action at each stage.
- Knowledge of Federal, State and Municipal laws and/or regulations that regulate the treatment of alcoholism and drug addiction, including:
- Confidentiality
- Client Rights
- Professional Code of Ethics
- Discrimination
- Knowledge of airborne and blood borne pathogens (TB, HIV, AIDS, HB) and risk reduction methodologies and the use of universal precautions.
- Flexibility to adapt to schedule changes and assumption of responsibilities not delineated in the job description which are related to work as a member of an addiction treatment team
SPECIFIC AREAS OF RESPONSIBILITY TO POSITION
- Work with 3rd parties to connect clients to appropriate aftercare setting, which includes:
Direct Case managers to provide documentation to outside parties when necessary for client continued programming (ie. Court attorney, FMLA letters or supporting documentation).
Stay in close communication with clinical/medical team to direct aftercare plan
Contact a potential referring agency and ensure necessary paperwork is sent to this agency.
Contact the client's family within 24 hours to discuss treatment recommendations.
Call in family to perform onsite interventions when necessary.
Coordinate with clients, family and receiving provider for successful transition - Assist with discharge planning and documentation, which includes:
Meet with client to review stay and planned discharge
Inform Medical/Clinical Staff of clients discharge date
Ensure client completes recommended LOC
Be available on call to assist case managers with coordinating the client’s treatment plan
Complete required discharge paperwork
Ensure paperwork is sent to the receiving facility
Coordinate transportation for client’s transition from the facility to the receiving facility if applicable. - Facilitate interventions with client’s displaying behavioral or mental instability and clients at risk for leaving Against Medical Advice (AMA)
Contact appropriate referents and family members for which agency has release of information (Informing these parties of client’s intent and soliciting their assistance either by phone or in person)
Make contact with Clinical Director for additional suggestions or recommendations to assist in redirecting client
Make contact with the Clinical Director to determine if clients are potentially a harm to themselves or others. - Prepare client for LOC transfer
Inform client and UR of residential LOC transfer
Ensure client’s chart is up to standard and attendance meets the criteria
5. Protect the privacy of all client information in accordance with GRNH, LLC’s privacy policies, procedures, and practices, as required by federal and state law, and in accordance with general principle of professionalism as a healthcare provider. Failure to comply with the company’s policies and procedures on client privacy may result in disciplinary action up to and including termination of employment.
6. May access protected health information (PHI) and other patent information only to the extent that it is necessary to complete your job duties. The incumbent may only share such information with those who have specific client information you have in your possession to complete their job responsibilities related to treatment, payment or other GRNH, LLC operations.
7. Expected to report, without the threat of retaliation, any concerns regarding GRNH, LLC’s policies and procedures on client privacy and any observed practices in violation of the policy to their supervisor.
*Calls with family and meetings with clients must be documented in the chart
*An email will be sent out Thursday to the clinical team summarizing any open cases
*All transports for the weekend must be communicated to Director of Ops Thursday morning
Salary : $60,000 - $75,000
