We are conducting a confidential search for a compassionate and experienced Licensed Vocational Nurse (LVN) to join a residential behavioral health and substance use treatment program. This individual will play a critical role in providing direct nursing care to clients participating in residential and detoxification services while collaborating closely with an interdisciplinary treatment team to ensure safe, effective, and patient-centered care.

Position Summary
The Licensed Vocational Nurse (LVN) is responsible for delivering direct nursing care, medication administration, withdrawal monitoring, nursing assessments, admissions support, phlebotomy, client education, and coordination with medical providers and pharmacies. This position works closely with the Nurse Practitioner, Medical Director, nursing leadership, and the clinical team to support the daily medical operations of a growing residential and detoxification program.
Key Responsibilities
Direct Nursing Care
- Provide direct nursing care for clients receiving residential and detoxification services.
- Complete nursing assessments, health screenings, vital signs, and ongoing medical monitoring.
- Carry out provider orders accurately and efficiently.
- Monitor clients throughout detoxification and communicate changes in condition promptly.
- Respond appropriately to medical concerns and emergencies.
- Maintain accurate, timely, and compliant electronic medical documentation.
Medication Administration & Management
- Administer medications and monitor for effectiveness and adverse reactions.
- Perform medication reconciliation upon admission and discharge.
- Coordinate prescription refills, prior authorizations, and medication procurement with pharmacies and providers.
- Maintain medication documentation, medication counts, and controlled substance records.
Admissions & Intake
- Review incoming medical records, hospital discharge summaries, and medication lists.
- Coordinate admission medications and ensure timely initiation of medical services.
- Support implementation of standing medical orders and detoxification protocols.
Clinical Procedures
- Perform phlebotomy and collect laboratory specimens within scope of practice.
- Conduct and document withdrawal assessments using CIWA-Ar and COWS.
- Coordinate laboratory processing and follow-up of results.
Medical Operations
- Support daily medication line operations.
- Maintain medication carts, treatment rooms, emergency equipment, and medical supplies.
- Assist with inventory management and ordering of medications and clinical supplies.
- Collaborate with Certified Medication Aides (CMAs) to ensure efficient workflow.
Client Education
- Educate clients regarding medications, withdrawal symptoms, chronic disease management, nutrition, smoking cessation, and overall wellness.
Collaboration & Compliance
- Participate in interdisciplinary treatment planning and team meetings.
- Communicate medical updates while maintaining HIPAA compliance.
- Assist with chart audits, documentation improvement, and quality assurance initiatives.
- Support regulatory readiness, infection prevention standards, OSHA compliance, and continuous quality improvement efforts.
Qualifications
Required
- Current unrestricted Texas Licensed Vocational Nurse (LVN) license.
- Minimum 3 years of nursing experience.
- Current CPR certification.
- Excellent communication, documentation, and organizational skills.
- Ability to work effectively within a multidisciplinary treatment environment.
Preferred
- Experience in behavioral health, substance use disorder treatment, detoxification, or residential care.
- Phlebotomy experience.
- Experience with CIWA-Ar and COWS assessments.
- Knowledge of Medication-Assisted Treatment (MAT).
- Familiarity with Joint Commission and Texas HHSC standards.
- Experience using behavioral health electronic medical record (EMR) systems.
