Position Overview
We are looking for dependable and compassionate Personal Care Service Providers (PCSPs) to join our team. In this role, you will provide hands-on, non-medical care and support to clients in their homes, assisting with daily living activities and promoting overall well-being.
You will work under the supervision of an RN, Director of Client Services, or Director of Operations while following an individualized Plan of Care.
What You’ll Do
- Assist with personal care (bathing, grooming, oral hygiene)
- Support activities of daily living (dressing, mobility, toileting, transfers)
- Prepare meals and assist with nutrition needs
- Perform light housekeeping (laundry, dishes, general upkeep)
- Provide medication reminders (no administration)
- Accompany clients to appointments when authorized
- Monitor and report changes in client condition
- Document visits using a mobile app (time, care activities, mileage)
- Maintain a safe, respectful, and comfortable environment
Qualifications
- High School Diploma or GED
- Completion of a state-approved PCSP training program (or willingness to complete)
- Must be at least 18 years old
- Valid driver’s license and reliable transportation
- Ability to meet state health requirements
- Dementia training (initial ongoing as required)

- Ability to follow care plans and document accurately
- Understanding of HIPAA and confidentiality practices
Physical Requirements
- Ability to lift, push, or pull up to 50 lbs
- Perform transfers and assist with mobility
- Stand, walk, bend, and move throughout shifts
- Ability to safely perform in-home care tasks
