Registered Nurse

Job Summary
The Hospice Registered Nurse provides compassionate, patient-centered care to individuals with terminal illnesses in their place of residence. The RN coordinates all aspects of the patient’s hospice care, ensuring symptom management, emotional support, education, and collaboration with the interdisciplinary team (IDT) while honoring the patient’s goals and end-of-life wishes.

Key Responsibilities1. Patient Assessment & Care Planning

• Perform initial and ongoing comprehensive assessments of hospice patients.
• Identify physical, emotional, psychosocial, and spiritual needs.
• Develop and update individualized Plans of Care (POC) in collaboration with the IDT.
• Evaluate patient response to interventions and adjust care as needed.

2. Symptom Management

• Implement and monitor interventions for pain, dyspnea, agitation, nausea, skin breakdown, and other end-of-life symptoms.
• Administer medications safely and educate patients/families about their use.
• Consult with the Medical Director for changes in treatment plans.

3. Case Management & Coordination

• Serve as primary case manager (if applicable): coordinate care among social workers, chaplains, aides, and therapists.
• Ensure continuity of care through timely communication with the team and documentation.
• Participate in IDT meetings and quality improvement activities.

4. Patient & Family Support

• Educate patients and their families about disease progression, hospice philosophy, comfort measures, and expectations at end-of-life.
• Provide emotional support and anticipatory guidance.
• Promote patient autonomy and honor advance directives and end-of-life preferences.

5. Documentation & Compliance

• Complete documentation in accordance with state and federal regulations (CMS Conditions of Participation) and agency policy.
• Maintain accurate medication profiles, visit notes, and POC updates.
• Report changes in patient condition immediately to appropriate personnel.

6. On-Call Duties (as assigned)

• Respond to urgent needs during assigned on-call shifts.
• Provide triage, crisis intervention, and after-hours visits as needed.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the state of practice.
• Minimum of 1 year of nursing experience (hospice or home health experience preferred).
• Valid driver’s license and reliable transportation.
• CPR certification (BLS).
• Strong assessment and critical-thinking skills.
• Ability to work independently and collaboratively with the IDT.
• Compassionate communication skills and comfort working with end-of-life care.

Physical & Work Requirements

• Ability to travel to patient homes, facilities, or hospitals.
• Ability to lift up to 50 lbs occasionally and assist with patient mobility.
• Exposure to various home environments and the emotional demands of hospice work.

Pay: $40.37 - $200.00 per hour
