Occupational Therapy Assistant (Home Health) – Flexible Schedule

Guardian Healthcare is a licensed Home Health Agency serving patients across Massachusetts. We are growing and seeking compassionate Occupational Therapy Assistants (OTAs) to join our dedicated home health team.
We offer flexible scheduling, supportive management, and meaningful one-on-one patient care in the comfort of each patient’s home. Full-time and per diem positions are available, with travel areas customized to your preferences.
Position Summary
As a Home Health Occupational Therapy Assistant, you’ll provide skilled therapy services under the supervision of a licensed Occupational Therapist to help patients regain independence, improve daily functioning, and enhance overall quality of life. You’ll assist with implementing individualized care plans and deliver high-quality therapy focused on safety, comfort, and long-term wellness.
This position reports to our Senior Clinical Manager for the Boston area.
Key Responsibilities
- Assist in implementing occupational therapy treatment plans developed by the supervising Occupational Therapist and ordered by the physician
- Provide therapeutic interventions to improve motor skills, coordination, and ability to perform daily activities
- Observe and report patient progress, responses to treatment, and any changes in condition to the supervising Occupational Therapist
- Educate patients and caregivers on adaptive techniques, home safety, and use of assistive equipment as directed
- Reinforce home exercise programs and therapeutic activities to support patient goals
- Document patient progress, outcomes, and interventions in compliance with agency and state standards
- Collaborate closely with supervising Occupational Therapists, nurses, physical therapists, care coordinators, and other members of the healthcare team
Qualifications
- Current Massachusetts licensure as an Occupational Therapy Assistant (COTA)
- Minimum 1–2 years of clinical experience (home health preferred)
- Valid driver’s license and reliable transportation
- Strong communication and documentation skills
