Position: Home Health Aide
Department: Guardian Health
Reports To: Director of Nursing
Status: PRN
Hours: Part-Time; schedule based on client and agency needs
Position Summary:
The Home Health Aide will report directly to the Director of Nursing. The home health aide monitors or reports changes in health status and provides personal care such as bathing, dressing, and grooming of the patient.
Responsibilities/Job Duties:
- Assist patients with personal care activities such as bathing, grooming, dressing, and toileting.
- Monitor patients’ condition by observing physical and mental condition, intake and output, and exercise.
- Assist with meal preparation, feeding, and medication reminders.
- Perform light housekeeping duties such as laundry, washing dishes, and changing bed linens.
- Provide companionship and basic emotional or psychological support.
- Maintain accurate records of services performed and report changes in patients' conditions to a nurse or supervisor.
- Follow the care plan outlined by the care team and participate in updating care plans when necessary.
Additional Expectations:
- Understand and comply with Guardian Health policies and procedures.
- Perform additional duties and responsibilities as assigned.
- Maintain confidentiality as related to client information under HIPAA regulations regarding Protected Health Information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.

Required Qualifications:
- High school diploma or equivalent.
- Certification from a state-approved Home Health Aide training program.
- Current CPR/First Aid certification
- Previous experience in home health or personal care preferred.
- Valid driver’s license, reliable and safe vehicle, state minimum required insurance, and a clean driving record are required.
- Strong communication and interpersonal skills.
- Physical stamina and the ability to assist clients with mobility.
- Compassionate and patient demeanor.
- US work authorization (Required)
Work Environment:
- Work is performed in clients' homes and may involve travel between client homes.
- Exposure to bloodborne pathogens and infectious materials is expected; appropriate PPE training and equipment will be provided.
- Occupational Exposure Risk: Category I – Routine Exposure.
- May involve exposure to infectious diseases and bodily fluids.
- Requires bending, lifting, standing for extended periods, and the ability to assist with mobility.
