Position Summary:
The Care Coach position has been created to lend support to caregivers that are caring for a family member with complicated needs. As a care coach, you will go to the homes of our caregivers and help them find resources, offer support, and build good will. The goal of a Care Coach is to make 2 visits per quarter and build support for the caregiver. Encouraging self-care and helping the caregivers set goals for themselves. This position will support Cluster J (Hancock, Shelby, Rush, Fayette, Union, Franklin, Decatur, Monroe, Brown, Bartholomew, Lawrence, Jackson, Jennings, Ripley, Dearborn, Jefferson, Switzerland and Ohio counties).
Key Responsibilities:
· Make home visits as required per state guidelines to ensure compliance.
· Maintain records of visits
· Communicate with the Care Coach Supervisor
· Travel within the assigned territory to see caregivers in their homes.
· Coordinate visit times with the caregiver to ensure they are scheduled at mutually convenient times and commit to honoring scheduled appointments—cancelling only in true emergencies.
· Document notes from visit in required apps on computer/ phone.
· Attend weekly team meetings (virtually).
· Perform other duties as assigned.

· Maintain confidentiality as related to client information under HIPAA regulations regarding Protected Health Information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
Education:
· High school diploma or equivalent
Qualifications:
· Strong understanding of the emotional and physical demands placed on caregivers.
· Experience working with medically complex populations.
· Must be detail oriented.
· Demonstrate good organizational skills and time management skills.
· Must have the ability to travel on a weekly basis as this role is a field-based position.
· Must have an active driver license and reliable transportation with insurance to drive throughout the day / territory.
· Ability to effectively communicate with patients and families.
· Strong critical thinking and problem-solving skills.
· US work authorization (Required)
· Must have your own computer/ iPad or tablet and cell phone.
