Your day as a Transitional Assistant Coordinator

Communication is key; you'll maintain regular updates with case managers and attend transition meetings to support participants before their discharge.
Additionally, you’ll be responsible for tracking all communications, documenting direct services, and ensuring timely submission of reimbursements.
