We are seeking a registered nurse with heart. The work we do can bring hope and healing to those that need it most... right in the comfort of their own homes.
Our ideal registered nurse candidate will self-direct home visits and support our paraprofessionals. Our RNs advocate for better health through interventions and recommendations.
Schedule: Monday through Friday schedule, times are flexible, some weekends and/or evening are appreciated.
Responsibilities include:
- Creating daily client visit schedules based on needs and requirements.
- Documenting according to contract and organization policy and procedures.
- Reporting findings, recommendations, and concerns to care team accordingly.
- Creating, reviewing, and updating care plans for assigned clients.
- Act as liaison between clients, direct care workers and management.
- Provide education, training and coaching to caregiving staff during placement and supervision visits.
- As schedule allows take on routine nursing visits as client needs dictate or managers request such as medication fills, BP checks, catheter changes etc.
- In urgent situations, be willing to provide hands on personal care for clients in need.
- Perform other related duties as assigned.
Prior home care experience required.
Prior supervisory experience helpful.
