Responsibilities
- Assist clients with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques.
- Monitor patients’ vital signs, observe changes in health status, and document findings accurately in care plans while adhering to HIPAA regulations to protect patient privacy.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote proper nutrition.
- Support clients with medication administration under supervision, ensuring timely doses and proper documentation.
- Maintain a clean and safe environment by performing light cleaning duties and ensuring hygiene standards are met at all times.
- Provide personal transportation assistance for appointments or errands, supporting independence within the community.
- Engage in patient observation to identify signs of dementia, Alzheimer's disease, autism spectrum disorder, or other neurodevelopmental or developmental disabilities, reporting concerns promptly.
Experience
- Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred.
- Demonstrated knowledge of patient care techniques including patient monitoring, first aid, CPR certification, and use of assistive devices like Hoyer lifts.
- Experience supporting individuals with dementia, Alzheimer's disease, memory care needs, or behavioral challenges associated with neurodevelopmental disorders.
- Familiarity with care plans development and implementation for residents with intellectual disabilities or developmental disabilities support needs.
- Proven ability to communicate effectively with clients and team members while respecting residents’ rights and maintaining confidentiality under HIPAA guidelines.
- Hands-on experience assisting with ADLs such as grooming, feeding, mobility assistance, and personal transportation in a home or community setting.
- Ability to perform heavy lifting when necessary and ensure safety during patient transfers or manual handling tasks.
