Position Overview:
We are seeking a Registered Nurse (RN) to serve as a Case Manager / Admissions Nurse.
This is a full-time, salaried position focused on admissions, case management, and patient care in a home health setting. This role is ideal for a nurse who values independence, flexibility, and meaningful one-on-one patient care.
Compensation & Benefits:
- Salary starting at $75,000 for qualified candidates. Final compensation will be determined based on experience and discussed during the interview process.

What You’ll Do:
- Complete Start of Care (SOC) visits and patient admissions
- Develop and manage individualized patient care plans
- Coordinate care with physicians and interdisciplinary team members
- Perform routine nursing visits as needed
- Provide clinical oversight to LPNs and collaborate with aides
- Maintain accurate and timely documentation
Work Area:
Assignments are typically based on your location, primarily within Montezuma or La Plata County, with travel kept reasonable and local whenever possible.
What We’re Looking For:
- Active Colorado Registered Nurse (RN) license
- Strong clinical judgment and assessment skills
- Excellent communication and organizational skills
- Reliable transportation and willingness to travel within the service area
- Home health experience preferred, but not required
