Hospice / Medical Social Worker – Day Shift
Job Type: 13-Week Contract
Location: Albany, OR
Start Date: Immediate
Pay Range: $50–$53 per hour (All-Inclusive)
Shift: Days (8:00 AM – 4:30 PM), No Weekends
Position Overview
We are seeking a qualified Hospice/Medical Social Worker to support patients and families during end-of-life care. This role provides in-home and facility-based support, including skilled nursing facilities, assisted living residences, and adult foster homes across multiple counties. The focus is on psychosocial support, care planning, grief assistance, and coordination of community resources.
Key Responsibilities
- Conduct psychosocial assessments for hospice patients and families.
- Develop care plans and collaborate with interdisciplinary hospice teams.
- Provide counseling, emotional support, and resource guidance.
- Assist with crisis intervention, grief counseling, and end-of-life planning.
- Travel to patient homes and partner facilities as required.
- Document all services accurately using EPIC.
Qualifications
- Required: Oregon LMSW or CSWA license.
- Preferred: Oregon LCSW.
- Required: Active BLS (Healthcare Provider Level) certification.
- Experience in hospital, hospice, or medical social work preferred.
- Valid driver’s license, reliable personal vehicle, and insurance required.

Pay: $47.24 - $53.00 per hour
Expected hours: 36 per week
